Toby Clark, RPh, MSc, FASHP, FFIP serves as a Lead Surveyor for the Accreditation Services Division of American Society of Health-System Pharmacists. Mr. Clark is also a medication systems consultant and an adjunct associate professor of pharmacy practice at South Carolina College of Pharmacy and consultant to the Director of Pharmacy at the Medical University of South Carolina. Previously, he served as (Adj) professor of pharmacy practice and director of hospital pharmacy services at the University of Illinois at Chicago Medical Center and College of Pharmacy before his retirement.
Clark is a long-time member of the American Society of Health-System Pharmacists, the South Carolina Society of Health-System Pharmacists, and the American Institute of the History of Pharmacy. He is a past officer of the Southeastern Michigan Society of Hospital Pharmacists and the Society of Hospital Pharmacists of Greater Kansas City. Mr. Clark has served two terms as Chair of the ASHP Practice Management Group (now Section of Practice Management) and has participated as a delegate to the ASHP House of Delegates for the states of Texas, Missouri, and Michigan. In 1998, he was recognized as a Fellow of ASHP.
He is a past North American Vice President and Treasurer of the Section of Hospital Pharmacy of the International Pharmacy Federation (FIP). In 2003, Clark was awarded the Donald E. Francke Medal for his contributions to international pharmacy and in 2013 awarded an elected Fellowship in the FIP. An avid medical/pharmacy historian, he is a member of the Dean Robert Wilson Medical History Society of Charleston and President of the MUSC Waring Library Society. Mr. Clark is a past chair of the University Health-System Consortium Pharmacy Council and Executive Committee and is a founding member of both the Oncology Section of the Board of Pharmaceutical Specialties and the Novation Pharmacy Executive Committee. In 2012 he was award the John W. Webb Lecture Award and Visiting Professor at Northeastern University, School of Pharmacy. In 2007 he was recognized for Distinguished Service by the ASHP Section of Informatics and again in 2013 received the Distinguished Service Award by the ASHP Section of Practice Managers.
He was born in Indiana and received his bachelor's degree in pharmacy from Ohio Northern University. He completed an ASHP-accredited general residency from Bronson Methodist Hospital in Kalamazoo, Michigan, and a Master of Science degree in clinical pharmacy from the Schools of Medicine and Pharmacy at Wayne State University. He has held positions as director of pharmacy and faculty member at the University of Missouri at Kansas City as well with several pharmacy management corporations. He was a faculty member at the University of Houston, College of Pharmacy prior to service at the University of Illinois at Chicago.
Toby’s professional interests are focused in the safety, quality and productivity of the medication system; training pharmacy residents; and the development of leaders. A former lecturer on topics of pharmacy/medication system informatics and computerized prescriber order entry, he spearheaded formation of the ASHP Section on Informatics and founded the group's POE/CPOE listserv, now operated by ASHP. He has been a consultant to scores of hospitals in the United States.
He strongly advocates hospital pharmacists serving patients boldly, through leading the safety and quality of medication systems.
Sara J. White, MS, FASHP is currently a primary faculty member for the ASHP Education and Research Foundation’s Pharmacy Leadership Academy (PLA) and Leaders Innovation Masters Series (LIMS) programs and a pharmacy leadership coach.
She graduated from Oregon State University with a bachelor's degree in pharmacy. After graduation, she worked at the University of Oregon Medical Center and Emanuel Hospital in Portland for two years. She then earned her Master of Science degree at The Ohio State University and completed an ASHP-accredited residency at The Ohio State University Hospitals and Clinics.
Sara served as associate director, director of clinical education, and professor at the University of Kansas Medical Center and School of Pharmacy for 20 years. She was the director of pharmacy at Stanford Hospital and Clinics and clinical professor at the University of California, San Francisco, for 11 years before taking an early retirement. She has served as an ASHP Foundation Scholar-in-Residence. She is in her third three-year term as a corporate board member for Omnicell.
White has served as president of the Greater Kansas City Society of Hospital Pharmacists, Kansas Society of Hospital Pharmacists, and American Society of Health-System Pharmacists. She has also served ASHP as chair of the House of Delegates, a board member, and a council member. She has received multiple awards for her contributions to pharmacy, including the Kansas Hospital Pharmacist of the Year Award, Kansas Pharmacist of the Year Award, Harold N. Godwin Lecture Award, Lambda Kappa Sigma Vanguard Leadership Award, Clifton J. Latiolais Award, John A. Webb Lecture Award, Ohio State Jack L. Beal Post baccalaureate Award, ASHP Distinguished Leadership Award and the Harvey A.K. Whitney Lecture Award.
In addition to developing, implementing, and leading comprehensive computerized clinical and distributive pharmacy services, which included computerized prescriber order entry and unit-based drug distribution automation, Sara has trained over 90 pharmacy residents and taught numerous pharmacy students. Sara has published and spoken widely, especially on the topic of leadership.
